1Enter what you know
Open Verify a Provider and type the provider's name and state. If you have their NPI number or license number, add it — the more you give us, the faster and more certain the check.
2We check the official sources
Eazelo looks the provider up in official public records — the national NPI registry, state medical license boards, and other government sources. This usually takes under a minute; tougher lookups can take a few.
You'll see each source get checked off as we go. Nothing is guessed: every answer comes from a real record, and we show you which one.
3Read your proof report
When the check finishes you get a report that says whether the provider was verified, what each source said, and links to the official records we used. You can share the report link with your team or download it.
If something couldn't be verified automatically, the report says exactly what's missing — and our review team can take it from there.
